This is omnios bloody.  I don't think it's related to this issue/driver. 
The M1015 was flashed to IT firmware when I first acquired it.  All 6
drives are Seagate ST1000NM0001 (nearline 1TB SAS drives).  Four drives
are firmware revision 0002 and two are PN04.  I wrote down the 2 good
vdevs and the 1 bad one, but that is on my desk at home, so I can't check
that against the firmware revisions right now...
